YOUR PRIMARY-CARE HOME
More than a sick visit. A plan for everyday health.
Bring these concerns to the conversation. Our team confirms what can be assessed here, which tests are useful, and when you need another service or specialist. This is not a promise that every procedure is available in-house.
Prevention & annual visits
Age- and history-appropriate cancer screening, cardiovascular risk, bone health, immunization-record review and shared health goals. Testing is individualized—not a universal annual panel.
Everyday illness & symptoms
Discuss non-emergency respiratory symptoms, urinary concerns, digestive symptoms, skin concerns and minor aches. Staff confirms whether the clinic can assess your problem or another setting is safer.
Long-term condition care
Blood pressure, cholesterol, diabetes, thyroid concerns and medication review, with follow-up plans and referral coordination. Ask about the scope of care for asthma, kidney disease or other ongoing conditions.
Weight, food & movement
Sustainable weight care, familiar foods, activity, sleep and tobacco cessation. Medicines are considered when appropriate—not guaranteed by a questionnaire or visit.
Women’s & men’s health
Menopause, sexual-health concerns, appropriate screening, and hormone discussions based on symptoms and medical history. Procedures and specialist services require confirmation or referral.
Mood, sleep & life transitions
Discuss anxiety, low mood, sleep concerns, caregiving stress, healthy aging and fall prevention. Arrange appropriate mental-health or sleep-specialist care when needed.
Medication & care coordination
Review prescriptions and supplements, reconcile overseas medicines, discuss refill needs, and coordinate after a hospital stay. No automatic refills, controlled-substance promises or specialist replacement.
Care that fits your circumstances
Adult primary care, telehealth when appropriate, language-support requests, immigrant-parent visits and self-pay options. Pediatric visits, pregnancy care, procedures and occupational forms must be checked with staff.
Call 911 for emergencies. Vaccination administration and immigration physicals are listed separately as coming soon, not current appointment options.
