HEALTH SCALES · A CONVERSATION STARTER

Check your health status.

Start with what you notice. Choose a questionnaire, understand its limits, and request an appointment when you are ready. For adults in Frisco, Plano, Allen and McKinney—not a diagnosis or an all-clear.

Know what a health check can—and cannot—tell you

These 10 tools are a curated selection, not a clinical ranking. PHQ-9, GAD-7 and CDC-based prediabetes scoring use published screening methods. Hormone, sleep, weight and habit checklists are custom, guideline-informed conversation aids—not validated diagnostic scales. No overall “health grade” is calculated. Green/orange/red are labeled result cues, not diagnostic or emergency-triage categories. Published scales retain their scores; custom checklists do not acquire invented risk cutoffs.

Answers stay in page memory and are not sent to the clinic or attached to appointment requests. The clinic does not monitor responses. Low scores cannot rule out illness. Call 911 for chest pain, severe breathlessness, stroke symptoms or immediate danger. In the U.S., call or text 988 for a mental health crisis.

How we cross-check these tools

PHQ-9 and GAD-7 question wording, two-week timeframe, answer weights and severity bands were compared with the published forms and scoring manual on October 13, 2026. The daily-function question is not scored. CDC age, BMI, activity and history weights—including the lower Asian BMI threshold and cutoff of 5—were checked against CDC scoring instructions. Sources are linked on each tool.

Sleep, movement, tobacco, weight and menopause guidance was compared with the linked NHLBI, CDC, NIDDK and NICE resources. The testosterone checklist retains its Endocrine Society guideline reference; that source could not be freshly retrieved during this check. Technical checks are not independent clinical validation or physician review. Custom tools remain conversation aids. Safety guidance is shown instead of a routine PHQ-9 score for any positive self-harm answer; drowsy-driving concerns receive a prompt warning.

Begin directly after the disclaimer—no age-eligibility or immediate-danger gate. Select radio answers and use Continue; the prediabetes form retains the age band required by its published score. Results explain their color, method and limits and invite you to talk to your doctor.

ADDITIONAL MEASUREMENT TOOLS

Put your numbers in context.

These calculators supplement the questionnaires; they do not estimate your lifespan or heart-attack probability.

ADULT BMI CALCULATOR

BMI (kg/m²)

Categories use the unrounded value. Body composition, waist measurement and medical history also matter.

Discuss weight & metabolic health

BMI screening ranges

For adults only; not for pregnancy. BMI can be misleading in athletes, frailty, fluid retention or altered body composition. The graph ends at 40; values above that remain displayed numerically.

Formula: weight in kilograms ÷ height in meters squared. US inputs use 1 inch = 2.54 cm and 1 pound = 0.45359237 kg.

Waist-to-height ratio

Measure midway between your lowest rib and hip bone after breathing out naturally. Use the same units for waist and height.

Units

NICE categories: 0.4–under 0.5 healthy central adiposity; 0.5–under 0.6 increased; 0.6+ high. Not an overall health diagnosis.

Non-HDL cholesterol

Total cholesterol minus HDL cholesterol estimates cholesterol carried by potentially atherogenic particles. Use values from the same lab report.

Report units

Do not enter LDL instead of total cholesterol. This tool does not calculate LDL or convert Lp(a) between mg/dL and nmol/L.

HORMONE HEALTH · YOUR NEXT CONVERSATION

Feeling different?
Start with what you notice.

A guided check-in for men’s testosterone concerns and women’s perimenopause or menopause symptoms. One question at a time, with clear next steps.

A conversation guide—not an HRT eligibility score.

The 0–15 symptom-impact total is a custom, non-validated summary of five answers. It cannot diagnose hormone deficiency, estimate treatment benefit, recommend a dose or tell you that HRT is safe.

Answers stay in this page, are not saved, and are not attached to appointment texts or lab requests. Reload or restart to clear them.

Evidence & scoring method

Each symptom is rated 0 (none), 1 (mild), 2 (moderate) or 3 (severe). We add the five ratings, without diagnostic cutoffs. Medical history and fertility answers guide the discussion; this tool does not recommend a lab package. The clinical guidance—not this custom score—is evidence-based. No endorsement by the source organizations is implied.

Sources checked September 10, 2026. Tool requires clinic review before use in clinical decision-making.
